More About Registration and Payment of Fees. There are two ways to register and remit fees:
1) Print registration form and mail in with payment (check or money order).
2) Register and pay online / electronically. Print registration form and voucher (if used) and scan. You can scan pages into Microsoft Word and then save as an Acrobat pdf. Send an email to firstname.lastname@example.org and attach your document. Indicate you wish to pay electronically, and specify your paypal email address. To pay electronically, we’ll send you a simple invoice via Paypal. Dr. Palmere will review your child’s registration form and call you to confirm all details before sending a Paypal Invoice.
